Overview
At HHC Health and Home Care our Hi Tech Program consists of a team of highly trained professionals, available to administer intravenous therapy to patients in the comfort of their home. This care offers patients a safe and effective alternative to hospitalization or other institutionalization. |

Admission/Eligibility
Patients wishing to participate in the program must have adequate home facilities to support Hi-Tech procedures, such as:
Electricity for lighting and electrical pumps, running water, and access to a telephone. A physician must order all care, and the physician and/or designee must be available by telephone for consultation. HHC Health and Home Care nurses can initiate client/family training in the hospital before the patient is discharged. The patient and significant others, if available, can be taught how to carry out the therapeutic regime. Additionally, the nurse provides emotional and technical support to the patient and significant other, as they learn new skills and attain competence. A comprehensive plan to manage potential emergencies will be established. |

Services

To promote coordinated, optimal care, HHC Health and Home Care employs a team approach. Services provided by the team may include: |

Demonstration and education of skills required to participate in program; |
| • |
Case management by IV- nurses available to explain details of treatment being provided, answer questions related to plan of care, and troubleshoot problems before they arise; |
| • |
24-hour/7-day-a-week on-call service; |
| • |
Access to all services offered by HHC Health and Home Care, including: physical, speech and occupational therapies; medical social work; certified home health aides; nutrition counseling; respiratory therapy and referrals to other community services. |

Coordination with other service providers to obtain necessary medical supplies and pharmaceuticals. |

Available Therapies: |

Antibiotic - Patients can be treated at home for a wide range of infectious diseases, including osteomyelitis, endocarditis, and lyme disease. |
| • |
Hydration - therapy for clients experiencing dehydration due to hyperemesis or other gastrointestinal symptoms as a primary diagnosis or secondary effect of treatment. |

Nutrition - Services are available for patients receiving total parenteral nutrition and enteral feedings. |
| • |
Pain Management - Patients with end-stage cancer, neurological diagnoses, or other conditions requiring pain control can be maintained at home with a continuos infusion pump to reduce pain and enhance their quality of life. |

Other Intravenous Therapies - Intravenous Immunoglobulin, Factor 8 and Iron chelation are available within a physician odered plan of care to clients of HHC Health and Home Care. | |
