Overview

The Long Term Home Health Care Program is the component of HHC Health and Home Care where a range of professional and supportive services are provided at home to patients who anticipate needing care for an extended period of time.

Admission/Eligibility

For admission to the program the following criteria must be met:
 
 The patient resides in Manhattan and the Bronx;
 The patient wants services and is motivated to participate in their own care;
 The patient's medical, social and environmental needs can be met safely at home;
 The patient is otherwise eligible for placement in a residential health care facility;
 The patient is under the care of a physician;
 The patient's condition necessitates intermittent professional health services;
 The patient has Medicaid, is eligible for Medicaid or elects
 to pay privately;
 The cost of services do not exceed 75% of the caps set by Medicaid for placement in a Residential Health Care Facility;
 The Department of Social Services authorizes services.

Speech and Language Pathology
Audiology Services
Social Work
Nutrition Counseling
Respiratory Therapy
Personal Care Workers
Home Health Aides
Homemakers
Housekeepers

Additional services such as: Transportation; laboratory services; medical supplies and equipment; telephone emergency response system; 24 hour on-call-service; social day care; respite care; home maintenance tasks and chore services are available.
