Overview

At HHC Health and Home Care our HIV/AIDS program is designed to assist patients living with HIV or AIDS maintain or improve the quality of their lives. To achieve this goal, patients are provided with clinical services targeted at helping them to: continue treatment; learn about and live with the wide range of HIV-related symptoms; manage the side effects of treatment and strengthen ways of coping with the stresses and concerns related to living with the illness. HHC Health and Home Care provides individually tailored services that enable patients to receive needed care in the comfort of their homes.

Admission/Eligibility

  For admission to the program the patient must:
   
Have a physician-documented diagnosis of positive HIV status or AIDS;
Be under the care of a physician;
Have a positive HIV viral load test;
Have a CD4 test below 500 or CD4 percentage below 29%;
Reside in Manhattan, Queens, or the Bronx;
Be motivated to receive home care services and participate in their plan of care.

Services

  To promote coordinated, effective care, HHC Health and Home Care employs a team approach.

Services provided by the team may include:
   
Skilled Nursing;
Telehealth Visits (nursing visits through an interactive audio visual monitoring system);
Social Work Services/Individual Counseling;
Physical, Occupational and Speech Therapy;
Nutrition Services;
Respiratory Therapy;
Durable Medical Equipment.

he program is designed to:
   
Reduce hospital length of stay;
Reduce the frequency of re-hospitalization;
Provide an alternative level of care for patients who might otherwise require nursing home placement;
Increase adherence to difficult treatment regimes;
Provide services to patients with HIV/AIDS who are uninsured/underinsured;

Assist individuals with HIV/AIDS reach maximal health, safety and independence within the community setting.
