The House Calls program puts the LATEST monitoring technology in the patient's home and provides patients with nursing consultation and outstanding personalized care, education and disease management.
When a patient is admitted into the House Calls program, a small modem is placed in the patient's home and connected to a glucometer, blood pressure cuff, scale or other monitoring device ordered by their physician. Each day, the user obtains their values, pushes one button and all their readings are sent to a telehealth nurse and triage team via a simple 800 toll free phone line.
Once the information is sent, the Telehealth team is alerted to any readings outside the range that has been ordered by their doctor. Results which are out of predetermined parameters are addressed promptly with patients and physicians and patient education is initiated to assist patients in maintaining their values within acceptable ranges. In some situations, doctors may need to adjust medications or order additional ones. And, the House Calls team doesn't just pay attention to abnormal results. Positive reinforcement is the key to program success and patients enrolled in the program get frequent phone calls and letters praising them for their efforts to effectively manage their disease!
